To secure the University Certification of Education form from the Master of Social Work Program at Chicago State University (IDFPR documents), you must have completed the MSW program at Chicago State University.

1. Using Adobe Sign, complete the (IDFPR) ED form. No further action is required. If approved you will receive CSU's signature and seal on the document you submitted.
2. Visit the  Website for more information or need more detailed instructions.

The form will be mailed to the address you submitted.

If you prefer to pick-up your form, email your request to: mswverify@datsumoki.net,

• Provide your name, phone number and email address. You will be notified when the forms are ready. The forms will be available for pick-up from the Office of the Registrar.
